Diagnostic laboratory services furnished by an independent laboratory are covered under medical insurance if the laboratory is an approved Independent Clinical Laboratory.

An independent laboratory is the one that is independent both of the attending or consulting physician's office and of a participating Medicare hospital or a non-participating hospital that meets at least the requirements for coverage of emergency services. A consulting physician, as distinguished in one providing clinical laboratory services, is defined as one whose services include history taking, examination of the patient and, in each case, furnishing to the attending physician an opinion concerning diagnosis or treatment. A laboratory that is operated by or beneath the supervision of a hospital which does not meet at least this is of a crisis hospital or of the organized medical staff of this type of hospital is known as to be an independent laboratory. The laboratory which a physician or a group of physicians maintains for performing diagnostic tests in connection with his own or the group practice wouldn't normally be considered an "independent laboratory."

An out-of-hospital laboratory is ordinarily presumed to be independent unless there is written evidence establishing that it is operated by or beneath the supervision of a hospital which meets the conditions of participation or of the organized medical staff of this type of hospital. A laboratory which serves the hospital patients and is operated on the premises of a hospital which meets the conditions of participation is known as to be subject to the oversight of a healthcare facility or its organized medical staff and hence, for purposes of application of the appropriate license and health and safety standards beneath the law isn't an independent laboratory.

A clinical laboratory is really a laboratory where microbiological, serological, chemical, hematological, biophysical, cytological, immunohematological, or pathological examinations are performed on materials derived from the human body, to provide information for the diagnosis, prevention, or treatment of a disease or assessment of a medical health insurance plans and condition.

C. An approved independent clinical laboratory is one which is approved by the Secretary of Health, Education, and Welfare as meeting the specified conditions for coverage beneath the program. These require that:

A. where State or applicable local law provides for licensing of independent clinical laboratories, the laboratory is either licensed under such law or is approved as meeting the requirements for licensing laboratories; and B. Such laboratories meet the safe practices requirements prescribed by the Secretary of Health, Education, and Welfare.
